CLAIMS REPORTING
If
an accident occurs:
-
Be sure to write down names, addresses, and license numbers
of persons and witnesses involved.
- Notify
the police immediately.
- Report
the accident to us as soon as possible by calling TOLL
FREE 1-800-333-2860.
- Do
not admit liability, authorize repairs or discuss your
accident with anyone except our representative or police.
- Call
your agent for advice or instructions if needed.
Loss Reporting

All
claims are reported to 1-800-333-2860. The Claims
Support Operators will take the information from the
insured and input the information into the computer.
We
have Claims Support Operators on duty from 7:00 a.m.
to 10:00 p.m. CST/CDT Monday through Friday. We have
Claims Support Operators on duty Saturday and Sunday
from 8:00 a.m. to 5:00 p.m.
Due
to the volume of calls, there may be times when a
caller will be connected to our voice messaging system.
They will be asked to leave their name, a phone number
where they can be reached (must include area code),
and their policy number. The Claims Support Operators
will return their call as soon as possible.

(claimant):

and approximate time of the loss?
- Location
of the loss?
- How
did the loss/accident occur?
- Was
a police/fire department notified?
- Case
or report number?

- Vehicle
and driver involved?
- Correct
address and phone numbers, specifically numbers where
they can be reached during the work day?
- Is
their vehicle driveable? If not, where is the vehicle?
- Area
of Damage
- Color
of vehicle or license number?
- Owner
of vehicle, if not the insured?
- Were
seat belts worn?
- Injuries
and where treated?
- Passengers'
name, address, phone number, Age? Seat belts worn? Injuries
and treatment facility?
- Make
and model of other vehicles involved?
- Area
of damage to other vehicles?
- Color
and license number?
- Name,
address, phone number and insurance information of owner
of other vehicles.
- If
owner of vehicle was not the driver, what is the driver's
name, address, phone number?
- Names,
addresses, phone numbers, injury information on any passengers?
Further
questions for the the claimant:
- Our
insured driver and vehicle?
- Is
our insured vehicle drivable?
- Area
of damage on insured vehicle?
- Any
injury or treatment facility for our insured?
- Names,
addresses, phone numbers, injury information on any passengers
in our insured vehicle?
- Make
and model of the claimant vehicle?
- Is
their vehicle drivable?
- Area
of damage?
- Name,
address, phone number, and insurance company information
of owner of vehicle, if not the driver?
- Passengers'
names, addresses, phone numbers, injury information?

After
reporting the loss information, the insured will be
given a claim number. He/she should keep this available
for future reference. The caller is also given the
name of the claims representative who will be handling
their claim, as well as their 4-digit extension number.
We
have a separate 800 number for calling to check on
existing claims. This number is 1-800-333-2861. You
will hear an automated attendant advising you to press,
1, 3 and enter the 4-digit extension number of their
claims representative.
